- Fax us your printed and completed form(s) or bring them with you to your appointment.
Payment Options
At Hasse Family Chiropractic we strive to provide the exceptional chiropractic care. We are able to do this by working with all insurance companies. Currently Hasse Family Chiropractic is in network with Landmark, Blue Cross and Blue Shield, and United Healthcare and their group cooperative plans. If this is not your insurance there still could be chiropractic coverage under your plan. If you are not sure about your chiropractic coverage call your insurance member information on the back of your card. If you would like to opt out of billing your insurance specific time of service discounts will be provided. We also allow and accept payment at the time of service including HSA accounts, major credit cards, cash, and check options.
